First-Year Student-Athlete Medical Forms
As an integral part of our preventative health care efforts, all student-athletes must complete additional athletic pre-participation medical requirements before they will be allowed to participate in any team practice. These requirements are set by the NCAA and are separate from the requirements for admission to the university. All forms and documentation must be submitted by August 1st.

-Â Â Physical Examination: Performed within 6 months from the start of your team's season.
-Â Athletes must complete Health Center requirements (click the above link) prior to participating in intercollegiate athletics.
Geneseo Sports Medicine Requirements
STEP 1 -Â Athletic Trainer System

Click on the link below, which will take you to the Athletic Trainer System log-in page. For both username and password type "new" then click login. You will be directed to the athlete information page in which you create your own username and password for future use. If you have any trouble logging in please email Jeremie Stearns at stearns@geneseo.edu.

STEP 2 -Â Â General Information
- Complete all required information and any additional information you may wish to enter. At the bottom of the page click "Save Athlete Information"
STEP 3 -Â Â Insurance
- Click on "Insurance" tab at the top of the Athlete Information page
- *When completing insurance information DO NOT input Social Security numbers
- *Athletes must bring a copy of their insurance cards to campus and must upload a copy to the ATSystem.Â
- To add a new insurance, click "add." Once you have completed the required information, click the blue checkmark at the bottom of the window to save.
- To add an image of your insurance card, scan or take a picture of your card, save it on your computer as an acceptable file and upload the image by clicking "select" at the bottom of the add or edit insurance window.
STEP 4 -
  Emergency Contacts
- Click on "Contacts"
- To add a new emergency contact, click "add." Once you have completed the required information, click the blue checkmark at the bottom of the window to save.
STEP 5 -Â Â Geneseo Medical History Update and Informed Consent
- Click on the "Forms" tab. Then select the "Geneseo Medical History Update" under the dropdown list, then click "new."Â
- Answer each question and give explanations to "yes" answer where applicable.
- Read and complete concussion agreement
- Read and complete informed consent
- After completing all questions, sign your name in the box provided - IF YOU ARE UNDER 18 YOUR PARENT OR GUARDIAN MUST SIGN FOR YOU
STEP 6Â -Â Upload Sickle Cell test results to ATSystem
   New NCAA Requirement. Documentation must be obtained from a health care provider
STEP 7 - Baseline Concussion Testing Download SWAY medical app (balance and cognitive assessment)
- Baseline concussion test will be administered with your respective team upon arrival on campus
Physical Examination Requirement
While the university requires all incoming students to have a physical within the last year, the NCAA requires all incoming varsity student-athletes to have had a physical within 6 months from the start of their sports season.
- If you are unsure when your first practice is, please contact your coach.
Instructions
- Follow the link to the Lauderdale Health Center Health requirements for first year students. Follow their instructions on submitting a physical.
- If you have already submitted a physical to the Lauderdale Health Center, check that it is within the 6 month period.
- If your physical is not within the NCAA required 6 month period you will have to obtain a new physical from your personal physician and resubmit to the Lauderdale Health Center
- If you are unable to obtain another physical due to insurance limitations or some other circumstance you may contact the Lauderdale Health Center to schedule a sports physical. However they may only have limited availability over the summer months.Â
NOTE: YOU WILL NOT BE ALLOWED TO PARTICIPATE IN VARSITY ATHLETICS AT SUNY GENESEO UNTIL ALL OF THE ABOVE MEDICAL REQUIREMENTS HAVE BEEN COMPLETED.
We will begin officially reviewing student-athlete medical requirements after August 1st, however we may make periodic reviews over the summer. If you are missing any of the required information, we will let you know what you are missing.
